Do You Actually Have Plantar Fasciitis?

Plantar Fasciitis is one of the most common problems that affect runners, jumpers and anyone who is on his or her feet all day. Bureau of Labor statistics say that tens of thousands of people suffer from plantar fasciitis and similar Repetitive Stress Injuries (RSIs) every year.

Any kind of “itis” (like plantar fasciitis) actually has a medically precise meaning, which is “inflammation”. Here are some quick and easy tests you can run to see whether you have plantar fasciitis, or if it's really a different condition like plantar fasciosis (the “osis” here indicates an actual degeneration of the fascia).

1. Did your pain stem from some injury or trauma? Or did it develop slowly over time? For example, if you jumped down from something that was a little too high and then felt a sudden pain in the bottom of your foot, you may have plantar fasciitis. But if you're a runner and started noticing a slight pain that has typically gotten worse or worse, then probably you're suffering from plantar fasciosis, not fasciitis.

2. Have you “fixed” your pain once (or twice … or three times), only to have it rear its ugly head again in a few weeks or months? Inflammation is generally a short-term response to some sort of injury or infection in the body. If you get sick or hurt yourself for some reason, infection can help your body heal itself. But once that job is finished, the inflammation should go away. (Remember the last time you got a splinter in your finger? Once you got it out, it didnt take long for the pain to fade away, did it?) If you have taken ibuprofin or other pain relief medication, given the problem area a rest, thought that it was healed … only to have the pain reoccur once you started back with your activity, well, chances are that you do not have plantar fasciitis.

3. Has the pain been there consistently for more than just a few weeks? Similar to the above point, if you suffer from long-term pain in one or the other of your plantar fascia, and especially if you have not injured or gotten an infection there, chances are pretty plantar rather than any “itis”.

4. Is there any swapping in the area? Is it red? Hot to the touch? These are three out of the four stereotypical symptoms of inflammation, and people have known about them since at least the time of ancient Greece. If the painful area is not hot, or red, or swollen, it's almost impossible that it could be inflamed. Once again, this means you do not have plantar fasciitis. Instead, the odds are overwhelmingly that it's plantar fasciosis, which is going to require a completely different approach to make it heal.

Of course, quite a few legitimate cases of plantar fasciitis do exist, but people (and this includes most doctors) have a tendency to confuse plantar fasciitis with plantar fasciosis. Take a few moments to think about the points raised in this article, then decide for yourself which one you have.

Should Kids’ Running Shoes Have Raised Heels and Support?

I do not pretend to be a pediatric, podiatrist or orthopedic specialist, but I think this question about kids' running shoes needs to be raised.

Too many running shoes for kids on the market today are simply smaller versions of the same kinds of shoes that the company sells to adults. They have raised horses, and lots of cushioning and support. I suspect they're not quite as durable, because the company knows the child will grow out of them before putting in the same kind of mileage that serious adult runners do, but the point is they still treat children's feet as if they were small grownups .

That concerns me, because in the adult market there's a growing trend towards barefoot running. Often “barefoot” does not mean totally barefoot, but wearing a minimal type of shoe such as Vibram Five Fingers or Nike Free. These protect the bottoms of feet from wear and tear of running on concrete, broken glass and rusty nails, but do not provide any support or cushioning.

This makes it easy for the runner to land on their forefoot or midsole, as many experts advise.

Ordinary running shoes demand running by landing on your heel. I ordinarily wear Brooks Beast – a style of shoe designed to give a lot of support to flat feet and to prevent pronation. I had not paid attention before, but I do land on my heels. When I tried to change to a forefoot landing, it looked very odd and unnatural, and I could not keep it up.

Yet, just go out to a field of grass and run barefoot. You will automatically land on your forefoot, because landing on your heel without the cushion cushioning you're used to is quite painful.

The type of running shoe we've gotten used to wearing since Nike first came out with them in 1972 has a raised heel, and lots of support and cushioning.

There's a growing belief among runners and orthopedic experts that this kind of shoe is not protecting their feet at all, but rather creating more injuries than they prevent. According to studies, up to seventy or eighty percent of all runners – weekend jocks as well as serious ultramarathoners – suffer a running injury every year.

At least one runner out there claims to have used foot exercises to improve his arch, going from a flat-footed size 13 (like me), to a well-arched 9 or 10.

This may not work for those of us who have a lot of flatfooted years. But it seems misguided these days to keep children's feet boxed in.

A Few Common Myths About Plantar Fasciitis

When people start to experience pain in their feet, the first thing many of them do is go see a doctor. After all, doctors are expected to have the experts when it comes to any kind of body pain. But the reality is that yours is probably mistaken when it comes to plantar fasciitis / fasciosis. It's a pretty good bet that you've been told at least one of the following:

First Myth: NSAIDs like aspirin, ibuprofen, etc. are a good treatment for plantar fasciitis

Very often, doctors will prescribe Non-Steroidal Anti-Inflammatory Drugs such as aspirin and ibuprofen, and the reason is that they believe plantar fasciitis is a result of inflammation. But the most current research shows that this really is not true. Since the origin of the problem is not what the doctors believe it to be, naturally NSAIDs can not do much to help. If you've ever taken ibuprofen and so on for plantar fasciitis, chances are that you might have gotten some temporary pain relief … but it probably did not really do anything to help the underlining condition.

Stuff like tumeric and willow bark, which are usually recommended, function through the inhibiting of the production mechanisms for COX-2. What is “COX-2”? Well, it's an enzyme that controls some forms of inflammation and pain. But here again, what's generally called “plantar fasciitis” is usually actually plantar fasciosis, which is not an inflammatory condition. Any relief you get from these supplements is probably because of their effect on the actual pain reduction mechanism rather than the root plantar fascia condition.
